New Patients
HOW TO REGISTER:
When you register with the practice you will be asked to fill out a
registration form, and a medical history questionnaire, and you will be
offered a new patient appointment with a Doctor or the Practice Nurse,
to allow a brief history of previous illnesses to be taken and routine
tests can be carried out e.g. blood pressure, etc. Please take a sample
of urine with you.
ACCESS FOR THE DISABLED:
The practice has access suitable for disabled and wheelchair borne.
A foldable wheelchair is available to assist patients into the surgery
if necessary.
