PRACTICE AIMS
At the Dunedin Medical Practice we
aim to operate systems which make it as easy as possible for our
patients to access a doctor, nurse, or other healthcare professional.
This Practice is committed to providing the best possible quality
healthcare designed to meet the needs of its patients.
To this end we specifically strive to achieve the following standards:

You will be greeted in a
friendly welcoming manner in clean and comfortable surroundings
where SMOKING IS NOT PERMITTED.

You have the right to
confidentiality and to be treated with respect - if you wish to
speak to any member of staff privately please ask.

In the interest of your health
it is important that your understand all the information given to
you. No care or treatment will be given to you without your informed
consent and any questions you may have regarding your health will be
answered.

Help Us to Help You

Please cancel your appointment if not required.

Give the staff as much information as possible to
enable them to deal with your request efficiently.

Each 10 minute appointment is for one person only.
Make a separate appointment for each who requires to be seen.

Please phone for results after 2 pm.

Notify us of any change of address and/or phone
number promptly.

Please request home visits as early as possible.

Come to the Practice if possible. The Doctor can see
3 or 4 people in the Practice in the time it takes to make one home
visit.

Order repeat prescriptions in plenty of time.

Emergency appointments are 5 minute slots—please use
these appointments to discuss the urgent matter only.

YOUR RIGHTS AND RESPONSIBILITIES, INCLUDING KEEPING APPOINTMENTS
All surgeries are by appointment and these can be made by telephoning
the practice. Every available attempt will be made to fit urgent cases
into the next available surgery on the same day.
The Practice also has the authority to remove from the list any patients
who constantly abuse the appointments system.
Please telephone to cancel your appointment if no longer needed, so your
appointment can be allocated to another patient.
Please notify us of change of address and/or telephone number promptly.
Remember, you are largely responsible for your own health, and it is up
to you to take the advice and action recommended to you.

DATA PROTECTION ACT
The Practice is registered under the Data Protection Act. The use of
personal health information is an integral part of patient care.
Protection of personal health information is part of good clinical
practice, and is underpinned by the Common Law duty of Confidentiality,
the implementation of the Data Protection Act 1988, revised Codes of
Professional Practice, and the new European Convention on Human Rights.
Arrangements for use of Personal Health information should meet these
requirements, as well as the recommendations made in the Confidentiality
and Security Advisory Group’s Report.
You are entitled to access your clinical records held by
the practice. This should be done in writing stating exactly what
information you require. A charge of between £10-£50 may apply for this
request. Please ask the Practice Manager for a form.

Freedom of
Information (Scotland) Act 2002
Freedom of Information (Scotland) Act 2002 came into force on 1st
January 2005, and enables any person requesting information from a
public body to receive that information, subject to certain exemptions.
This is to encourage public authorities, (GP Practices) to be more open
and accountable and organise their information in an efficient and
accessible way. This excludes
personal data.

VIOLENCE AND
AGGRESSION
This practice operates a ZERO TOLERANCE policy of verbal or
physical aggression towards all practice staff. The practice has the
right to remove patients from the list because of persisting
unacceptable behaviour.

Confidentiality of Records
Practices undergo regular visits by external assessors, whose purpose is
to verify the process of the practices quality of care to the patient;
therefore they may need to view records of a random number of individual
patients. The visiting team adheres to a strict code of confidentiality.
If you do not wish your records to be inspected, then please contact the
Practice Manager.
